If you do not have to pay for your prescription, should you have the following exemption status:
You are under 16 years old
You are 16, 17 or 18 and in full-tine education
You are over 60
You have a valid maternity exemption certificate
You have a valid medical exemption certificate
You have a valid prescription pre-payment certificate
You have a valid War pension exemption certificate
You are named on a current HC2 charges certificate
You have been prescribed free-of-charge contraceptives
You get Income Support (IS)
You get income based Jobseekers Allowance (JSA (IB))
You are entitled to, or named on, a valid NHS Tax Credit Exemption Certificate
You have a partner who gets Pension Credit guarantee credit (PCGC)
You do not have to pay for your prescription, should you have the following exemption status:
Where to sign?
Part 1 If you do not pay for your prescription please tick the correct box. Part 2 If you pay for your prescription put the amount in the box, £7.40 per item. Part 3 Please make sure you sign the prescription in the correct box.
On receipt of your prescription it will be dispensed and despatched on the next working day.
